Job Summary: We are seeking a compassionate and dedicated Home Care Nurse Aide to join our team at Reference Home Care, LLC. The Nurse Aide will provide essential support to individuals in need of home health care services. The ideal candidate will be committed to delivering high-quality care, possess excellent communication skills, and demonstrate empathy and professionalism in all interactions.
Responsibilities:
- Personal Care Assistance: Assist clients with activities of daily living (ADLs) including bathing, grooming, dressing, and toileting, ensuring their comfort and dignity.
- Mobility Support: Help clients with mobility issues to move around their homes safely, utilizing mobility aids as necessary.
- Medication Reminders: Assist clients with medication management by reminding them to take prescribed medications at the appropriate times.
- Meal Preparation: Prepare nutritious meals and snacks according to clients’ dietary needs and preferences, ensuring compliance with any prescribed dietary restrictions.
- Light Housekeeping: Perform light housekeeping tasks such as laundry, dishwashing, and tidying up living spaces to maintain a clean and safe environment for clients.
- Companionship: Provide companionship and emotional support to clients, engaging in conversation, activities, and recreational pursuits as appropriate.
- Observation and Reporting: Monitor clients’ health status and report any changes or concerns to the supervising nurse or case manager in a timely manner.
- Documentation: Maintain accurate and up-to-date records of care provided, including daily activity logs, vital signs, and any incidents or changes in clients’ conditions.
- Collaboration: Collaborate effectively with other members of the care team, including nurses, therapists, and social workers, to ensure coordinated and comprehensive care for clients.
- Client Education: Educate clients and their families about self-care techniques, safety precautions, and strategies for managing chronic conditions to promote independence and well-being.
Qualifications:
- High school diploma or equivalent required; completion of a certified nurse aide (CNA) training program preferred.
- Current certification as a Nurse Aide in [State].
- Prior experience in home health care or a similar setting preferred.
- CPR and First Aid certification preferred.
- Strong interpersonal skills and the ability to communicate effectively with clients, families, and team members.
- Compassionate and patient-centered approach to care.
- Ability to work independently and adapt to changing situations.
- Reliable transportation and a valid driver’s license required.
Physical Requirements:
- Ability to lift and transfer clients safely, with or without assistance.
- Ability to stand, walk, bend, and kneel for extended periods.
- Good manual dexterity and hand-eye coordination.
Working Conditions:
- Work primarily takes place in clients’ homes, which may vary in terms of cleanliness, accessibility, and environmental factors.
- Flexible scheduling may be required, including evenings, weekends, and holidays, to accommodate clients’ needs.
- Potential exposure to infectious diseases and hazardous materials; adherence to infection control protocols and safety procedures is essential.
